How Churches Can Build a Care Pathway

Most churches want to care for their people well. The problem is rarely intention — it is infrastructure. Without a structured approach, pastoral care becomes reactive, inconsistent, and exhausting. The Care Pathway model offers a different vision: a proactive, layered system in which people can be matched with the level of care they actually need.

What Is a Care Pathway?

A Care Pathway is a series of clearly defined steps and resources that a church puts in place to help members navigate personal crises, ongoing struggles, and spiritual formation needs. Rather than funneling everyone to the senior pastor, a Care Pathway distributes care across trained volunteers, lay counselors, deacons, and ministry staff, with appropriate referrals to outside professionals when needed.

The Three Tiers of Care

Most Care Pathway models describe three tiers:

  • Tier 1 — Mutual Care: The informal network of the congregation — small groups, Sunday School classes, and friendships. This is where most biblical one-anothering happens.
  • Tier 2 — Lay Counseling and Pastoral Care: Trained lay leaders and deacons who can provide more sustained support, listen well, and offer basic biblical guidance.
  • Tier 3 — Professional/Specialized Care: Licensed counselors, biblical counselors, and pastoral supervisors who can address clinical-level needs, trauma, addiction, and other complex presentations.

Why Churches Need This

The data is clear: most people in crisis will go to a pastor before they go to a therapist. This is an enormous opportunity — and an enormous responsibility. When a church has no pathway beyond talk to the pastor, people either don’t get help, or they burn out the pastor, or they fall through the cracks at the referral point.
