"Do not be anxious about anything, but in every situation, by prayer and petition, with thanksgiving, present your requests to God." — Philippians 4:6
Anxiety is not first a thinking problem — it is a trusting problem. This worksheet invites you to trace the thought beneath the worry, the belief beneath the thought, and the desire beneath the belief — and then to bring all of it honestly before God.
Step 1 — Name What Is Happening
- 1
What specific situation, event, or thought is triggering anxiety right now?
- 2
What sensations are you noticing in your body? Where is the anxiety held physically?
- 3
On a scale of 1–10, how distressing does this feel? What does that number mean to you?
1. What specific situation, event, or thought is triggering anxiety right now?
2. What sensations are you noticing in your body? Where is the anxiety held physically?
3. On a scale of 1–10, how distressing does this feel? What does that number mean to you?
Step 2 — Examine the Thought
1. What is the most anxious thought that comes to mind about this situation?
2. What are you telling yourself about what will happen? Be specific.
3. What is the worst outcome you are imagining? How likely is that, actually?
4. If that worst outcome happened, then what? Keep following the thought — what does it believe about what you could not survive?
Step 3 — Trace the Belief Beneath the Thought
1. What does this anxious thought assume about God? (e.g. He is absent, He cannot be trusted, He doesn't care, He is not enough)
2. What does it assume about you? (e.g. I am alone, I cannot handle this, I must manage this myself)
3. Where did you first learn to believe this? Is there an older story underneath this present worry?
Step 4 — Bring It Before God
1. If you were to speak to God honestly right now — not a cleaned-up prayer, but the real one — what would you say?
2. What are you afraid God will not do, or cannot do, in this situation?
3. What would it mean for you to "present your request with thanksgiving" here? What makes that difficult?
Step 5 — Receive the Truth
1. What does Scripture say about this specific fear? Look up one passage and write it in your own words.
2. What would it look like to "think about these things" (Phil. 4:8) — true, honorable, just, pure, lovely — in this situation?
3. What is one concrete, small act of trust you can take today?
Closing Prayer Space
1. Write a short, honest prayer — not asking for the anxiety to go away, but asking God to meet you in it.
